Job Title: Adult Social Worker - Hospital Discharge Team

Location: South Shields, Tyne and Wear, United Kingdom

Salary: 24 per hour

We are seeking a dedicated and compassionate Adult Social Worker to join our Hospital Discharge Team in South Shields. As an integral part of our team, you will play a crucial role in facilitating the safe and timely discharge of adult patients from hospital to their homes or appropriate care settings.

Responsibilities:

  • Conduct comprehensive assessments of patient's social care needs, taking into account their physical, emotional, and psychological well-being.
  • Develop and implement personalised care plans in collaboration with patients, their families, and multidisciplinary health-care teams.
  • Coordinate discharge planning processes, ensuring seamless transitions for patients from hospital to their homes or alternative care settings.
  • Provide support and guidance to patients and their families, signposting to relevant community resources and services.
  • Advocate on behalf of patients to ensure their rights and preferences are respected throughout the discharge process
  • Collaborate closely with health-care professionals, including doctors, nurses, occupational therapists, and physiotherapists, to optimise patient outcomes
  • Maintain accurate and up-to-date records of assessments, care plans, and interventions, adhering to relevant legal and professional standards.

A strong Candidate will have:

  • Previous experience working in a similar role within a health-care or social care setting
  • Knowledge of relevant legislation, policies, and procedures related to adult social care and safeguarding.
  • Strong assessment and decision-making abilities, with a person-centered approach to care.
  • Excellent communication and interpersonal skills, with the ability to engage effectively with patients, families, and multidisciplinary teams.

Requirements:

  • Registered with Social work England
  • A Degree in Social work or Diploma in SW, CQSW, CSS
  • Right to Work
  • Enhanced DBS check, dated within the last 12 months
