Our Learning Disabilities Transitions Team plays a crucial role in ensuring a smooth and successful transition for young people as they navigate the challenges of adulthood.

Role and Responsibilities: As a Senior Social Worker in the Learning Disabilities Transitions Team, you will:

  • Lead and coordinate transition planning for young people with learning disabilities, working collaboratively with the individual, their family, and partner agencies.
  • Conduct assessments and develop person-centred transition plans that address the individual's needs, aspirations, and goals for adulthood.
  • Provide support, guidance, and advocacy to young people and their families, empowering them to make informed decisions and access appropriate services and support networks.
  • Work closely with education, health, housing, and employment services to ensure a holistic approach to transition planning and support.
  • Supervise and support less experienced team members, providing guidance and mentoring to enhance their practice and development.
  • Maintain accurate records and documentation, ensuring compliance with statutory requirements and council policies.

Requirements for this role

  • Qualified and registered Social Worker with relevant experience
  • Knowledge of legislation and regulations
  • Willingness to participate in continuous professional development.
