We are currently seeking a Care Coordinator to join our team in Manchester. As a Care Coordinator, you will play a crucial role in providing essential support to our clients, enabling them to lead fulfilling lives whilst managing a Support team in the feild

Requirements:

  • Experience in Mental Health, Autism, Learning Disability and Dementia
  • 1 year management experience or 2 years Care Coordinator experience
  • Level 3 Health and Social Care (Level 4 referable)
  • Drivers License

Responsibilities (Social Care):

  • Patient Assessment: Conduct initial assessments of patients or clients to determine their needs, preferences, and eligibility for specific services.
  • Care Planning: Develop personalized care plans in collaboration with healthcare professionals, clients, and their families. Ensure that care plans are tailored to meet the unique needs of each individual.
  • Communication and Coordination: Facilitate communication and coordination among various healthcare professionals, service providers, and support staff. Serve as a liaison between the patient, family members, and the healthcare team.
  • Resource Identification: Identify and connect patients or clients with appropriate community resources, support services, and healthcare providers.
  • Monitoring and Evaluation: Regularly assess and reassess the effectiveness of care plans. Monitor the progress of patients or clients and adjust care plans as needed.
  • Documentation: Maintain accurate and up-to-date records of patient assessments, care plans, and coordination efforts. Ensure compliance with relevant legal and regulatory requirements.
  • Education and Support: Provide education and support to patients, families, and caregivers regarding their care plans and available resources. Foster a collaborative and empowering relationship between the healthcare team and the patient
