Randstad is seeking a Qualified Social Worker to work within Lancashire County Council's Hospital Discharge Team. This full-time position offers a competitive pay rate of 37 per hour.

Responsibilities:

  • Facilitate the smooth and safe discharge of residents to suitable settings.
  • Implement various Discharge to Assess and re-ablement pathways to promote independence outside of the hospital environment.
  • Conduct assessments to identify immediate and long-term care needs, supporting individuals in achieving their care goals.
  • Collaborate with a multi-agency team to ensure effective, safe, and timely discharges across different hospital sites.
  • Conduct Mental Capacity assessments, participate in Best Interest Meetings, and attend MDT meetings as required.
  • Contribute to a Duty rota.

Requirements:

  • Registration with Social Work England
  • Enhanced DBS clearance
  • Social Work Degree or equivalent qualification
  • Experience working with adults
