Qualified Adults Social Worker | Transitions Team | Sheffield
- Qualified Adults Social Worker / Social Work Professional required to join a Transitions team
- Hybrid working
- Office base: Sheffield
- You must have a social work qualification and full SWE registration as well as relevant experience to be considered
- Pay rate 30ph
To be considered, it is essential that you have a recognised Social Work qualification (Social Work Degree or Diploma in Social Work) and full Social Work England registration as well as recent or significant relevant experience in a similar role.
The candidate should have experience working with challenging client groups.
Managing a case load and completing assessments, support plans and reviews with adults. Completing Court of Protection documents, best interests and safeguarding investigations. As part of the role to assess risk, explore suitable placements (Supported Living, residential and housing) alongside health professionals.
Must be experienced to deal with all aspects of the task. Require candidate to support the team and hit the ground running. Must be IT literate and ideally have Liquid Logic experience. If not then training will be provided.
Ideally should be available at short notice.
